Vietnam War 50th Anniversary Commemoration

The Cook County Department of Veterans Affairs would like to invite veterans to join us for the 50th Anniversary of the Vietnam War Commemoration. The purpose of this program is to honor and recognize the veterans that served during the Vietnam era from 1955 through 1975. The ceremony will take place at the Swan Pavilion inside Brookfield Zoo starting at 11:00 A.M. on Thursday, September 29th, 2016.

Those interested in attending must RSVP no later than September 15th. Veterans must complete the attached application and submit it via fax, email, or mail to Cook County Veterans Affairs. The contact information is provided below.

Cook County Veterans Affairs will call upon receipt of the applications to confirm information and to reserve a spot at the event, as spaces are limited. We will also like to recognize the wives of those Vietnam era Veterans who have died.

In order to be recognized, veterans will need to submit the following information:
• Vietnam era Veterans’ Name, Rank, Branch and Time of Service (click here for form)
• If widows, please provide the Deceased Vietnam era Veterans’ Name and Date of Death, including the same information listed above and adding the name of the spouse.
